We honor your right to confidential healthcare information. We’ll make every attempt to protect the privacy of your information so that it’s not heard, read or shared for any reason other than to promote the best healthcare outcomes.
We require signed authorization from the patient or legal guardian to disclose protected health information. You can request copies of your health information by calling our medical records department at (608) 489-8131 or faxing requests to (608) 489-4899.
