Swing Bed is a Medicare-covered program designed to bridge the gap between hospital and home. It's a personalized path to recovery, offering skilled care and rehabilitation in the comfort of a hospital close to home. Even if you received care elsewhere, Gundersen's Swing Bed program can seamlessly continue your journey towards healing.

Conditions

Recovering from a major health event takes time. You may no longer need the intensive care of a hospital, but you’re not ready to return home. Swing Bed acts as a bridge that offers the skills of providers, nurses and therapists as you regain your strength and independence.  

Swing Bed care can help if you’re healing from:

  • Cardiac events  
  • Illness
  • Injury
  • Joint replacement  
  • Stroke
  • Surgery  
  • Other conditions requiring skilled care

With Swing Bed care, you're not just getting medical treatment. You're getting the support and guidance you need to reclaim your life. Our goal is to help you regain your strength, independence and confidence so you can get back to living.

Your healthcare team develops an individualized recovery plan that focuses on rehabilitation and pain management. During your stay in Swing Bed, you can participate in:  

  • Occupational Therapy to restore daily living skills like bathing, dressing, grooming, cooking and basic housekeeping
  • Physical Therapy to help you regain strength, improve mobility, prevent falls and resume daily activities
  • Respiratory Therapy to provide education and support for oxygen and other needs
  • Speech Therapy to address communication skills, including expressing thoughts, reading and safe swallowing

Swing Bed eligibility

Even if you had surgery or were hospitalized elsewhere, you can transfer to Gundersen for Swing Bed care. You’re eligible if:

  • Your provider finds you need daily skilled care after your treatment in the hospital
  • You have Medicare Part A or another insurance plan that covers skilled care benefits
  • You're committed to actively participating in your personalized care plan and making progress towards your recovery goals
