Insurance, billing and financial assistance
We’re committed to helping you understand healthcare costs and how to pay for them.
If you have insurance, the best way to determine your out-of-pocket costs is to contact your insurance company directly. Gundersen Moundview accepts nearly all insurance plans, but deductibles and co-pays vary based on your plan.
We accept the following major plans:
- Anthem Blue Cross Blue Shield
- Dean Health
- Government insurances such as TriCare, Medicare A & B and WI Medicaid
- Group Health of Eau Claire
- Quartz
- Security Health Plan
- UMR
- United Healthcare
- Wisconsin Physicians Service (WPS)
- Most Medicare supplements
Whether you have insurance or not, a cost estimate tool is also available to help determine the potential cost of a procedure or treatment.
To discuss questions about a bill or set up payment plan options, please contact our financial advisor at (608) 339-8324. A financial assistance program is also available to those who meet eligibility requirements.
Interpreter services
We offer no-cost translation services to patients and their families who have limited English proficiency or who are deaf/hard of hearing. Interpreter signs are posted at our reception desks or ask our staff.
Cafeteria and vending
Room service is available to hospitalized patients and their visitors daily from 7 a.m. to 5:30 p.m. Visitors may also purchase food at our cafeteria window Monday through Friday from 8:30 to 9:45 a.m. and 12 to 1:30 p.m. Vending machines are located near the cafeteria’s dining room, in the hallway connecting the hospital and clinic lobbies, and in the emergency room waiting area.
Gift shop

Our gift shop is staffed by Partners of Gundersen Moundview volunteers. The shop is near the hospital’s main entrance and is open Monday through Thursday from 9:30 a.m. to 3:30 p.m. These hours may vary due to our volunteer workforce. The shop is closed on Fridays, weekends and holidays.
Inside, you’ll find a variety of items including candy/gum, unique gifts, decor, greeting cards, jewelry and more. Only cash and check payments are accepted. Proceeds from the gift shop help fund local healthcare needs such as equipment and initiatives.
Patient and family services
We offer patients and families help with:
- Discharge planning
- Advance directives
- Accessing area resources (information is also available through CommunityLink)
- Emotional, social, financial, family and health concerns

Medical records
We require signed authorization from the patient or legal guardian to disclose protected health information. When requesting records for someone other than yourself, you must provide legal documentation verifying legal guardianship, power of attorney, personal representative, etc. Biological parents may request copies of their minor child’s records if their parental rights have not been terminated.
To request medical records, email us at MDV-MedicalRecords@gundersenhealth.org. You may also call (608) 339-3331 and ask for our Medical Records department or fax the department at (608) 339-4435.
An easy way to access your protected health information (PHI) is through MyChart. Log in online to securely view your records, manage who sees your PHI or connect your account to other health systems.
Pharmacy

Our retail pharmacy, located within the hospital, serves all customers regardless of where they receive care. In addition to prescriptions, several over-the-counter medications and health products are available. For more information, visit Gundersen Moundview Pharmacy.
