Scholarship applications

Who can apply

Candidates may either be:

  • Graduating seniors of Adams-Friendship and Westfield school districts
  • Community members in Adams and Marquette Counties returning to school for healthcare-related training

How to apply

Candidates must be currently attending or preparing to attend post-secondary institutions and study in the area of healthcare.

Applicants must:

  1. Complete the online application
  2. Email an essay (no more than one page) describing your healthcare goals and why you have chosen a healthcare career
  3. Email three character references, each of whom must write a short letter of support
  4. Email your high school or college transcript, as applicable

Deadline

All completed applications must be received by Moundview Memorial Healthcare Foundation by 4:30 p.m. on April 15.

Notification

The scholarship committee will notify the recipient by email or phone. Please double-check to make sure your application includes your correct address, phone number and email address.

About our healthcare scholarship

A scholarship of up to $1,000 will be awarded to up to five recipients pursuing healthcare-related courses of study.

The scholarship will be paid in one sum, distributed mid-summer, after a first successful year of school, which consists of having a 3.0 (measured on a standard of 4.0 grade scale) or B average. Recipients may apply for the Gundersen Moundview Healthcare Scholarship up to two times but must continue to maintain a 3.0 GPA or B average to be eligible. The scholarship funds will be paid directly to the scholarship recipient, not to the post-secondary institution.

After the full year of study, the recipient must report by correspondence to the scholarship committee of their status and include an official transcript. All information submitted will remain confidential.

Determination

The scholarship committee will award the monies on the basis of the following:

  • Scholastic ability
  • Financial need
  • Reference recommendation

Scholarship committee

The scholarship committee will be a team of Moundview Memorial Healthcare Foundation board members, Gundersen Moundview Hospital and Clinics staff and community members.

Additional notes

A permanent file will be kept on each scholarship recipient and the money awarded.

The scholarship may be terminated if the recipient:

  • Drops or changes majors before payouts of any scholarship monies
  • Commits an act that would preclude the recipient from working in healthcare

The committee may seek additional information, as deemed appropriate from school and other officials, in making their scholarship selection.
