Honoring extraordinary nurses
The DAISY (Diseases Attacking the Immune Systems) Award recognizes nurses for their dedication to patients and families. It honors the legacy of Patrick Barnes, who passed away from an autoimmune disorder known as ITP.
The DAISY Award is the first of its kind to give patients, families and colleagues a way to express their gratitude for the compassionate care that nurses provide.

How to nominate
Who can I nominate?
You can nominate any Gundersen Boscobel nurse practitioner, registered nurse or licensed registered nurse in good standing.
How can I nominate a nurse?
Use our online form below, complete the form found at our locations or return our nomination form by mail.
How are winners selected?
Gundersen Boscobel’s DAISY selection committee reviews nomination ballots after a scoring rubric. Each May, all nominees receive a pin and certificate, while the honoree is gifted with a handcrafted statue during an organization-wide celebration.
