With MyChart, you can easily manage your care and stay connected with your care teams from a computer, tablet, smartwatch or mobile device. This private healthcare management tool improves communication between you and your providers and offers convenient self-service options.

Registration is simple and can be completed within a few minutes. Simply download the MyChart app to your Apple or Android device.

Virtual care options in MyChart
With MyChart, you can schedule and access video visits through MyChart, allowing you to see your provider more conveniently within our secure patient portal. Same-day and next-day appointments are available.
Proxy access for medical records
You can use MyChart to access the medical record of another family member, such as a spouse, partner, elderly parent, stepchild or close friend. This feature in MyChart is called "proxy access.”

How to set up proxy access
Complete a Proxy Authorization Form, scan it and email the form to mychart@gundersenhealth.org.
Granting access to someone
To grant someone else access to your medical record, log into MyChart. In the search menu under Sharing and Family Access, click Share My Record. Follow the prompts and click "Invite Someone." Then complete the online form.
You must have or set up a MyChart account to get proxy access to someone else's account. If you are not a patient but would like proxy access to another account, please contact our MyChart team.
Your child’s medical record
- As a parent, you can access your child's health record through MyChart until their 18th birthday.
- To request access to your child's record, log in to MyChart. In the search menu, under “Sharing and Family Access,” navigate to "Manage Family Access." Then, "Request Access to a Minor's Record."
- Complete the online form, and you're finished.
