Near our main campus is Gundersen St. Elizabeth's Care Center - Shields Avenue, a skilled nursing facility with 80 beds and a specialized Alzheimer's Unit for those with moderate to severe dementia. Whether it's a short-term rehabilitation stay or a long-term care, our compassionate caregivers offer holistic, person-centered care. In collaboration with our families, we develop an individualized care plan that includes medical and therapeutic care, meaningful activities, social connections and family involvement. The well-being of our residents is enhanced by our comfortable surroundings and amenities. Our care center is a Veterans Affairs (VA) contracted facility.

Our residents are rehabilitated to their highest level of independence, and we understand the importance of returning to their home safely, swiftly and doing what they enjoy.
