Near our main campus is Gundersen St. Elizabeth's Care Center - Shields Avenue, a skilled nursing facility with 80 beds and a specialized Alzheimer's Unit for those with moderate to severe dementia. Whether it's a short-term rehabilitation stay or a long-term care, our compassionate caregivers offer holistic, person-centered care. In collaboration with our families, we develop an individualized care plan that includes medical and therapeutic care, meaningful activities, social connections and family involvement. The well-being of our residents is enhanced by our comfortable surroundings and amenities.  Our care center is a Veterans Affairs (VA) contracted facility. 

Services

When setbacks happen, recovery matters most. Our professional team of nurses, therapists and providers is ready to help whether the hospital stay was planned or unplanned. Our residents are rehabilitated to their highest level of independence, and we understand the importance of returning to their home safely, swiftly and doing what they enjoy. 

If you can no longer live at home on your own, we’re here to help you in transitioning to long-term care. We invite you and your loved ones to visit our facility and meet our staff. We will assess your medical background and prioritize creating a care plan that meets your specific requirements and preferences.

Respite care can be relief and reassurance when a patient or caregiver needs it most.

Offering nursing services and no required length of stay, you can count on our team to provide quality around-the-clock care for 5 days or 5 weeks - it’s up to you.

Contact our social work team to discuss the direct admit process and options for financial coverage. Respite services are not dependent upon hospice enrollment and may be covered by Medicaid or private pay.
