- Saturday, Nov. 23, 9-11 a.m. at the Gundersen Onalaska Clinic, 3111 Gundersen Drive, Onalaska, WI 54650 (Education Center on the Lower Level). Please use the Lower Level/Sports Medicine entrance on the back side of the clinic. Parking is available near the Lower Level entrance or in the parking ramp.
What you will learn:
Sometimes, having conversations about your health and future medical choices can be overwhelming. This advance care planning workshop, led by trained facilitators, will guide you through the process and assist in the completion of a Power of Attorney for Health Care. A Power of Attorney for Health Care is a legal document that allows you to identify who you would want to make medical decisions for you if you were unable to speak for yourself and what you would want those medical decisions to be.
You are encouraged to invite the individual(s) you would choose as the person to speak for you, in the event that you are not able to make them for yourself.
This group facilitation is for all adults starting at age 18. All forms will be provided.
During this time, you will:
- Gain a better understanding about advance care planning and why it is important.
- Participate in reflective conversations that help identify your own goals and values.
- Learn the qualities to consider when selecting a health care agent.
- Complete a Power of Attorney for Health Care document that is individualized and best supports what is important to you.
- Leave with a plan in place!
